What Reports Do You Provide?
Professional reports serve as a comprehensive documentation that captures the assessment, progress and outcomes of a child's therapy journey. These reports are meticulously prepared by our therapists and are valuable tools for both parents and other healthcare professionals.
Our professional reports provide a detailed snapshot of a child's development, highlighting their strengths, challenges and milestones achieved during therapy. It also serves as an important tool to justify eligibility for funding support. They are structured to be clear, informative and accessible.
NDIS Funding Application & Review
We provide comprehensive reports to support your children’s NDIS Funding Application
A parent/care-giver session is required to discuss progress, goals and recommendations prior to commencement of the NDIS review report.
Progress Report
Upon request, we are able to provide a report on your child’s development and progress to educators, paediatricians, and other allied health practitioners.
The progress report may include recommendations on school readiness, referral to other services or whether further occupational therapy support is required.
School Observation Reports
This report is provided upon the completion of a school or kinder observation session, where your therapist assesses your child’s participation, self regulation, social interactions, attention, motor skills, handwriting skills and overall independence throughout the day.
It will also include specific strategies, therapy resources and customised accommodations tailored to your child’s unique needs for the educators to follow through.
